We are committed to transparency and your right to access your health information. Below you will find instructions for requesting your records, contacting our licensing authority, and filing a complaint.

How to Request Your Health Care Records

Under federal and Texas law, you have the right to access and obtain copies of your health care records. To request your records from our practice, please follow these steps:

  1. Submit a written request. You may send your request by email, mail, or by completing a request form at our office. While some requests may be accepted verbally, a written, signed, and dated request is recommended.
  2. Specify the records you need. Clearly describe which records you are requesting (e.g., "all records from January 2024 to present" or "treatment summary and discharge plan").
  3. Include identifying information. Provide your full legal name, date of birth, and contact information so we can verify your identity and locate your records.
  4. Be aware of fees. We are permitted by law to charge a reasonable, cost-based fee for copying and mailing records. We will not charge a retrieval fee.
  5. Timeline. We will respond to your request within 15 business days if records are maintained in an electronic health records system, or within 30 days otherwise. If additional time is needed, we will notify you.

How to File a Complaint

If you have a concern about the services you received, we encourage you to contact us directly so we can address your concern. If you wish to file a formal complaint, you have the following options:

Complaint About a Licensed Professional:

You may file a complaint with the Texas Behavioral Health Executive Council (BHEC) regarding professional misconduct by a licensee. Complaints must be submitted using the BHEC's official complaint form.
